Dear Mr. _Keppler:
The purpose of this letter is to inform you of a situation that occurred concerning a recent commitment made by the Donald C. Cook Plant to the NRC.
On December 7,1981, in the text of a letter ( AEP:NRC:0652) to Mr.
Harold R. Denton, Director, Office of Nuclear Reactor Regulation, United States Nuclear Regulatory Commission, from R.S. Hunter, Vice President, Indiana and Michigan Electric Company, a commitment was made to keep the Containment Instrument Room purge isolation valves 1-VCR-101,1-VCR-102,1-VCR-201, and 1-VCR-202 closed in operating modes 1, 2, 3, and 4.
All circumstances concerning the commitment and reasons for its origin' l
have been reported via LER 50-316/81-059/03L-0, dated January 6, 1982.
On December 30, 1981, however, the Containment Instrument Room purge valves were opened in Mode 4 at the beginning of an outage on Unit 1.
l The valves were open for a total of 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and-27 minutes, after which I
the unit entered Mode 5 where this restriction of the commitment did not

apply.
l The existing automatic trip functions of these valvrs (Containment Phase A Isolation or Containment Particulate or Radiogas activity-High) were c
operable during the time the valves were open and there were no adverse effects upon public health and safety.
Plant Management'was aware of the commitments in the letter (AEP:NRC:0642) which originated at the corporate offices in New York City, and an i
effort had been made to communicate the commitment to Operation's per-sonnel.
Because this message did not convey the complete intent of the commitment, operators opened the valves, which had been standard opera-ting procedure in the past.

Mr. J.G. Keppler February 11, 1982 Page 2 of 2 The affected Operations Department procedure, Instrument Room Purge, has been revised so that the Instrument Room purge isolation valves will not be opened in Modes 1, 2, 3, or 4, except as provided for during surveil-lance testing.
The entire matter discussed above concerns Unit 1 only.
