Interim Report dated December 11, 1981, from GLKoester, KG&E (KMLNRC 81-145), to GLMadsen, NRC Subj: Potential 10CFR50.55(e) Report, ESW Pipe Support Embed Plant Stud Failure

Dear Mr. Madsen:

On November 13, 1981, Kansas Gas and Electric Company made a telephone report (EWCreel, KG&E, to GLMadsen, NRC) regarding failed studs on an Essential Service Water pipe support embedded plate which resulted in the plate pulling away from the wall.

The Reference described that our Architect / Engineer was evaluating the Nonconformance Report that was tracking the resolution of this failure.

During the Architect / Engineer's evaluations, unfalled stud-to-plate welds were tested and exhibited ultimate capacities greater than those used as the basis of design. The preliminary indications are that the failure resulted from plate shrinkage distortion, which was caused by excessive weld me.terial deposits at the hanger attachment plate connection to the embed plate. The weld shrinkage due to overwelding was further amplified by the dissimilarity to plate materials (carbon steel embed plate and stainless steel hanger attachment plate).

A final report is due from the Architect / Engineer in late March, 1982.

After KG&E personnel have had a chance to review and evaluate this report, we will submit our findings to you on or before April 23, 1982.
