Dear Sir:

The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

Due to the extensive investigation and evaluation required, an Interim Report is attached.

It is now expected that this informa-tion will be finalized by July 6, 1982, at which time a complete report will be submitted.

POTENTIAL PROBLEM Pursuant to a General Electric 10CFR Part 21 Report, Bechtel has-

'been notified that.some of the type HMA auxiliary relays manufactured 4

from 1976 through June 1981 may have the following problem:

The length of-the uninsulated flexible leads connected to the move-able contacts exceeds the allowable dimensions and can result in reduced spacing between the leads and the coil circuits. This condition introduces the possibility of circuit connection between the flexible leads or the HMA contact-circuits and the adjacent coil circuit-terminals.

Bechtel Engineering has determined that the identified HMA relays may have been installed in safety related switch gear, load centers, and AC distribution panels, plus other non-safety related equipment.

APPROACH.TO AND STATUS OF PROPOSED RESOLUTION-Bechtel Engineering will provide Bechtel Contruction with a detailed list of equipment to inspect for these relays. Bechtel Construction to initiate Nonconformance Reports as applicable and obtain replace-ment relays from the General Electric Field Representative to facilitate the necessary replacements.

III.

PROJECTED COMPLETION OF CORRECTIVE ACTION AND SUBMITTAL OF THE I

FINAL REPORT Evaluation of this condition and submittal of the Final Report is forecast to be completed by July 6, 1982.
