Specifications Section 6.9.5.E and 3.14.6.2, the Sacramento Municipal Utility District hereby submits the following special report.

On January 4,1982, the hourly fire watch in the B Diesel Generator Room was not conducted from 1530 hours0.0177 days <br />0.425 hours <br />0.00253 weeks <br />5.82165e-4 months <br /> to 1924 hours0.0223 days <br />0.534 hours <br />0.00318 weeks <br />7.32082e-4 months <br />. The fire watch was required due to a non-functional fire barrier penetration seal.

The door to the room had been replaced with a new door and closed at 1530 hours0.0177 days <br />0.425 hours <br />0.00253 weeks <br />5.82165e-4 months <br />.

A cover plate was supposed to be installed over the normal door latching mechanism so that the door would be controlled only by its security latching device. This plate was omitted, so that when the door was closed it latched in a locked position. When the fire watch attempted to gain entry, the security latch opened, but the door latch was still holding the door closed. No key was available for this latch, and it took until 1924 hours0.0223 days <br />0.534 hours <br />0.00318 weeks <br />7.32082e-4 months <br /> to gain entry without causing damage to the door.

Normal fire detection and suppression equipment was operational in the affected room during this period of time.

There were no transients asscciated with this situation nor was public or plant safety affected.
