SAFETY INJECTION TANK 'AhlD REACTOR PUtiP SUPPORT RT INDICATIONS (D/N #23)

A) Potential Problem A review of NSSS radiographs submitted to the site prior to April 1981 resulted in the generation of a Nonconformance Report (NCR). The NCR disclosed problems with radiographic technique, radiographic documentation (reader sheets), and welds containing unacceptable discontinuities. A meeting held by Ebasco with the Supply System and Combustion Engineering resulted in additional radiography being per-formed and documentation corrections. The following areas remain to be corrected:

NCR Item Vessel ID Area 4 Safety Injection Tank #1 Seam WW WW-10 5 Safety Injection Tank #1 Seam WW-ll - WW-10 7 Safety Injection Tank #2 Seam WW WW-14 9 Safety Injection Tank #4 Safe Ends 11 Safety Injection Tank #3 Safe Ends Because of items 9 and 11 above, questions have been raised concerning the safe end welds on Safety Injection Tanks #1 and #2 for WNP-3 and all four Safety Injec-tion Tanks for WNP-5.

Note: When re-radiography of safe ends for tanks 3 and 4 was performed, indica-tions were noted in the radiographs that were not present in the original film.

As all eight (8) tanks were manufactured using the same process, Ebasco's conten-

_~ tion is that all (8) tank safe ends should be re-radiographed to assure that indi-( cations detected in tanks 3 and 4 were not created by the manufacturing process.

B)- Proposed Resolution

~

Obtain concurrence between Combustion Engineering, Supply System and Ebasco on ixtent of rework and additional radiographs and assignment of rework under ASME Section XI or III. After this determination is made, an implementation schedule will be developed.

C) Status of Proposed Resolution A meeting was held on January 11, 1982 to determine who will perform the rework.

As a result of discussions during the meeting, CE will rework items 4, 5, 7 and safe ends for tanks 3 and 4. Ebasco will have Contract 3240-224 perform re-radio-graphy on safe ends of tanks 1 and 2 and inform CE of the results.

Due to the apparent termination of Unit #5 and the equipment for this Unit being in CE's possession, the re-radiography of #5's Safety Injection Tanks 1-4 will be CE's responsibility.

D) Final Report Delay Ebasco, Combustion Engineering and Supply System review of the problem requires additional time to complete resolution.

Expected completion date of the final report is indeterminate because of the pro-blems stated ia (b), (c), and (d) above. An interim report will be provided by April 1,1982.
