This refers to the special safety inspection conducted by Ms E. Matson of this office on January 15, 1982, of activities at Cardiac Diagnostic

and Rehabilitation Center authorized by NRC Byproduct Material License o
No. 21-17101-01 and to the discussion of our findings with Dr. P. Atallah;~
at the conclusion of the inspection and with you by telephone'on January 28, 1982.
The inspection was an examination of activities conducted under your license
[
as they relate to radiation safety and to compliance with the Ccamission's 1
rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and i terviews with personnel.
In addition to the above areas, the inspection was conducted to obtain information regarding an allegation that Dr. P. Atallah is using Byproduct.
Material for nuclear medicine procedures and is not qualified. The inspec-tion revealed that Dr. Atallah is working under the supervision of Dr. Gunabalan and no items of noncompliance were identified with respect to use by an unqualified person.
During this inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in the enclosed Appendix.
A written response, submitted under oath or affirmation, 'is required.
The responses directed by this letter (and the accompanying Notice) ar,e' not subject to the clearance procedures of theOf fice of Management and Budget as required by the Paperwork Reduction Act of 1980,' PL-96-511.
