I. DESCRIPTION OF DEFICIENCY During a Bechtel repair operation to the Reae or Coolant Pump No. lA supplied by Combustion Engineering, imperfections (sand holes) were found'$n thrust

-bearing housing 8000-400-001. Repair attempt by grinding to remove sand holes disclosed-imperfections extended below .750" depth.

II. ANALYSIS OF SAFETY IMPLICATIONS ,

As per the attached Combustion Engineering letter V CE-15596 dated 12-4-81, this conditon is evaluated as not reportable since no foreign material remained in the " cavity" and the defect does not affect the . structural' integrity of the thrust bearing housing. Should this condition have remained undetected it would not represent a safety significant condition.

CE-KSB Engineering and Quality Assurance have reviewed DER #81-42 and have ,

the following comments:

The conditions described in NCR-NC-520 and idt hC-563 were addres' sed '

and documented (Attachment (1) by CE-KSB and' Glacier Metal Company LTD (Bearing Housing manufacturer) in November,1979, and again in August, 198T. These conditions were found not to be detrimental to performance of the uni.t6 CEsKSB's acceptance of the subject condition is based on (1) the def,4ct does not affect structural integrity, and (2) following a high pressurer flush of the area any possible imefgn material remain-ing in the Ocavity" will not become dislodged since' there is no appreciable oil velocity in the area during operation.

The disposition by C-E to grind .750" maximum depth was based upon incomplete information since Scot-Test LTD's UT Report (Attachment.(1))

shows porosity at depths greater than .750". CE-KSB has again reviewed this condition, and finds the component acceptable for use. However, rework must be perfonned as the cavity has been excavated. Rework should consist of " cosmetic" grinding to remove rough and feathered edges t

with blending into surrounding area, high pressure flush to remove for,eign material. The final prepared area shall be sealed with Synobel- -
