Dear Sir:

The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

Due to the extensive investigation and evaluation required, an Interim Report is attached. It is now expected that this information will be finalized by March 24, 1982, at which time a complete report will be submitted.

J INTERIM REPORT - DER 81-54 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT 1 I. Potential Problem.

During an internal cleanliness inspection of Steam Generator #2, a linear indication in the nozzle outlet (Bechtel Weld W-001) was discovered by visual inspection. The indication is approxi-mately h" in length and appears to run parallel with the root bead in the root and heat affected zone. Prior to the visual discovery, the following NDE's were completed as satisfactory on the subject weld, e RT Inspected to ASME III, August 13, 1980 e PSE UT Test Per ASME XI, August 20, 1980 o PWHT Completed, September 24, 1980 II. Approach To and Status Of Proposed Resolution

-Bechtel Construction is currently taking the necessary steps to investigate and correct this indication by alternately perform-ing a light grinding operation followed by a PT inspection until the indication is removed.

III. Projected Completion of Corrective Action and Submittal of the Final Report Evaluation of this condition and submittal of the Final Report is forecast to be completed by March 24, 1982.
