Faulkenberry to Mr. R. S. Leddick, NRC Inspection at Washington Nuclear Project Nos. 3 and 5, (WNP-3/5).
The referenced letter reported the results of the NRC inspections conducted by Mr. D. P. Haist on October 5-9 and 26-30,1981, of acti-vities authorized by NRC Construction Permit Nos. CPPR-154 and 155.
One Item of Noncompliance (81-17/05) concerning Ebasco's failure to follow conditional release procedures was identified.
Attached is a report detailing corrective / preventive actions taken for the subject noncompliance. As a result of achieving full compli-ance on January 15, 1982, the Supply System considers the violation to be satisfactorily resolved.

Page 1 ATTACHMENT I Nuclear Regulatory Commission - Noncompliance (50-508/81-17/05) 10CFR50, Appendix B, Criterion V, states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualita-tive acceptance criteria for determining that important activities have been satisfactorily accomplished."
WPPSS Quality Assurance Requirement No. QAR 2.1, Rev.1, Paragraph 3.5.4 states:
" Representatives of the CM at the Project Site shall be in com-pliance with the Supply System-approved CM Quality Assurance Program and the applicable Project interface requirements."
Ebasco Project Site Procedure No. MM-ll-10, Rev. O, " Conditional Release of Material, Equipment for Construction," Paragraph 3.2.3 states "If con-ditional release of the item modifies the disposition or evaluation of the NCR, (originator) initiates change to correct NCR in accordance with PSP-QA-7-3 (Reference 4.1)."
Contrary to the above, Conditional Release Request No. 54-06 was issued on June 4,1981 to release fourteen (14) nonconforming containment pene-tration assemblies for partial installation prior to completion of dis-position of nonconformance report no.12966 in accordance with vendor procedure no. 3240-36529, Rev. 1.
Vendor procedure no. 3240-36529 spe-cifies methods for resetting containment penetration bellows assemblies prior to any installation activities.
Nonconformance report no.12966 was not revised to provide engineering direction to the installing con-tractor to enable resetting of the containment penetration bellows assem-blies after partial installation.
This is a Severity Level V Violation (Supplement VI) applicable to Unit No. 3.
Corrective Actions Taken The following corrective actions have been taken to assure compliance with 10CFR50, Appendix B, Criterion V:
1.
Engineering has redispositioned the NCR to provide explicit direction to the installing Contractor for resetting of the containment pene-tration bellows assemblies after partial installation.
2.
Engineering has rewritten the vendor's procedure to describe the meth-od of checking bellows assembly alignment after partial installation and the method for resetting as necessary.
This procedure has been made a part of the installing contractor's installation procedure.
Action Taken To Prevent Recurrence 1.
The Condition Release Originator has been re-instructed to follow Pro-cedure No. MM-ll-10, Paragraph 3.2.3-NOTE and to initiate revision to

2.
Engineering has been instructed to promptly revise any NCR issued to the installing Contractor whenever there is a change to the disposi-tion.
Date of Full Compliance 1.
The NCR was redispositioned on December 2,1981.
2.
The Engineer's approved procedure for resetting of bellows on par-tially installed mechanical penetrations was incorporated into the installing contractor's installation procedure on December 11, 1981.
3.
Actions taken to prevent recurrence were completed and documented on January 15, 1982.
