-Appendix NOTICE OF VIOLATION
' William Beaumont Hospital License No. 21-01333-02 As a result of the inspection conducted on January 12-14, 1982, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),.the following violations were identified:
1.
10 CFR 35.21(a)(3) requires full calibration measurements to be per-formed on each teletherapy unit at intervals not exceeding one year.
Coi. wary to the above, as of the date of this inspection, the last full calibration of your teletherapy unit was performed on August 14, 1980.
This is a Severity Level V violation (Supplement VII).

10 CFR 35.22(b)(2) requires monthly spot check measurements on each teletherapy unit to include the congruence between the radiation field and the field indicated by the light beam localizing device.
Contrary to the above, your teletherapy unit was last checked for congruence on August 27, 1981. Moreover, a licensee representative stated that the congruence on you teletherapy unit is usually checked quarterly.
This is a Severity Level V violation (Supplement VII).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-l pliance will be achieved. Under the authority of Section 182 of the Atomic I
Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Consideration may be given to extending your response time for good cause showa.
