Interim Report - DER 81-47 A 50.55(e) Potentially Reportable Deficiency Relating to Grout Patch Voids in Unit 1 Main Steam Support Structure Floor Slab File: 82-019-026 D.4.33.2

Dear Sir:

The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

Due to additional time being required for evaluation, an Interim Report is attached.

It is now expected that this information will be finalized by February 17, 1982, at which time a complete report will be submitted.

Very truly our,

f INTERIM REPORT - DER 81-47 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT #1 I.

Potential Problem An existing grout patch in the bottom of the Elevation 100'-0" floor slab of the Main Steam Support Structure was found to be hollow and cracked. Four (4) exploratory holes were drilled and the patch material was found to be soft and not bonded.

The grout patch was found to extend under an embed plate and under approximately one-half of an embedded unistrut. The 3'-6" concrete slab was placed on April 20, 1979 (Placement #1M003-U1).

This area was apparently repaired with pneumatically placed grout soon after the forms were stripped on April 21, 1979. PVNGS DER 79-12 covers the past problems associated with pneu-matically placed grout.

Discovery of this particular defective grout patch is considered an isolated case since the corrective action taken in regard to DER 79-12 is considered effective.

II.

Approach To and Status Of Proposed Resolution The cardition is being analyzed to determine whether there is enough embedment in sound concrete of the anchor bolts on the embed plate for the required load, and whether the surrounding concrete is sufficient for the reinforcing steel to resist the forces in the slab under all postu-lated loading.

There is a single conduit attached to the embedded unistrut within the area of the grout patch.

III.

Projected Completion of Corrective Action and Submittal of the Final Report Evaluation of this condition and submittal of the Final Report is forecast to be completed by February 17, 1982.
