Indian Point Unit No. 2 Inspection Report 50-247/81-20 ENCLOSURE A Item of Non-Compliance 50-247/81-20-01 We concur with the NRC findings with respect to Step Off Pad (SOP) deficiencies; however, it should be noted that these particular SOPS were located in areas with heavy traffic and significant construction activity.

To reduce wear and tear of the paper SOPS and to prevent reocurrence of these deficiencies, translucent plastic covers have been placed on approximately 80% of Unit #1 SOPS and on 100% of Unit #2 SOPS.

The remaining Unit #1 SOPS are expected to be recovered by December 31, 1981.

To ensure proper maintenance in the future, daily inspections have been initiated for heavily used Unit #1 and Unit #2 SOPS.

All other SOPS are inspected on a weekly or monthly basis.

Inspections are documented on check off lists which are submitted for review to the Radwaste General Supervisor.

Item of Non-Compliance 50-247/81-02-06 With respect to the item on procedure adherence and appropriate procedure changes,

a. temporary procedure change was in effect at the time of the NRC inspection.

This procedure change included the revised metnodology observed by the NRC inspectors for cal-culating mci content of LSA drums.

However, due to an oversight by Con Edison personnel, this procedure change was not attached to the copy of Health Physics Instruction (HPI) 9.11 submitted to the inspectors for review.

With respect to proper recording of survey information, the survey infornation should have been recorded in attachments to three health physics instructions (HPI 9.11, 9.12, and 9.15).

Contrary to the requirements, the survey information was only recorded in the attachment to HPI 9.15, and all sections of the attachment were not completed.

To correct this situation, the survey information is no longer required to be completed in the three health physics instructions, but rather one document.

Personnel have been in-structed to complete all sections of the form.
