Dear Mr. Kellogg:
This letter refers to the violation in the subject inspection report i
which states:
"As a result of the inspection conducted on October 26-28 and November-3-6, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
Farley Technical Specification 4.0.5 requires that inservice inspec-l tion be performed in accordance with ASME Boiler and Pressure Vessel Code (B&PV),Section XI.

Paragraph IWB-2500 of ASME B&PV,Section XI requires that the I
weld surface and one wall thickness on each side of the weld be examined for circumferential pipe welds.
2.
Paragraph IWA-2221 of ASME, B&PV,Section XI specifies MT inspec-tion in accordance with Article 7 of ASME B&PV,Section V.
Para-graph 5 of ASME Specification SE-109, referenced for amplified details by Article 7, requires that dry magnetic particles be applied by lightly dusting a small quantity over the surface and that excess particles be removed with a gentle air stream.

Mr. Paul J. Kellogg December 17, 1981 Page Two Contrary to the above, Inservice Inspections were not being performed in accordance with ASME B&PV,Section XI in that:
1.
On November 5,1981, for welds 4 and 5 on ISO ALA-128, one wall thickness on the valve side of the welds was not being liquid penetrant inspected.
2.
On October 27, 1981, magnetic particles were being applied to weld 2-24 on ISO ALA-217 using a method resulting in a heavy stream of particles being applied to the surface and the excess particles were being removed by the examiner blowing the surface with his mouth."
Admission or Denial The above violation occurred as described in the subject report.
Reason for Violation The violation cited in the NRC inspection report was caused by personnel error. The basis of the first example of the violation was the misinterpretation of Westinghouse Procedure ISI-11, Liquid Penetrant Examination Procedure, by the NDE inspector. The second example was due to the failure of the NDE inspector to utilize the correct apparatus for applying and removing magnetic particles.
Corrective Actions Taken and Results Achieved Corrective steps taken on the first example of this violation in-cluded the completion of the inspection on the valve side of welds 4 and 5.
Also, all previous inspections by the examiner involved were checked to identify any other pipe-to-valve welds. Two other pipe-to-valve welds were identified. These welds were re-examined.
The correct interpretation of Westinghouse Procedure ISI-ll and ASME Section XI by the other NDE examiners was confirmed.
In regard to the second example of this violation a sample population of 5 out of 11 of the welds previously inspected by MT, were rein-spected to verify the adequacy of the MT technique.
This reinspec-tion, which included weld 2-24 on IS0 ALA-217, yielded satisfactory results. All NDE examiners and their supervision were informed of this violation and were briefed on the correct method to apply and remove magnetic particles in accordance with ASME Section XI and Section V.
Corrective Steps Taken to Avoid Further Violations To avoid further violations of this nature, the Inservice Inspection
Mr. Paul J. Kellogg December 17, 1981 Page Three Administrative Procedure FNP-0-AP-57 will be revised to include spot checks of inspector technique.
Date of Full Compliance The revision of FNP-0-AP-57, the Inservice Inspection Administrative Procedure, will be completed by January 15, 1982. All other correc-tive action is complete.
