APPENDIX A Gibbs and Hill, Inc.
Docket No. 99900524/81-04 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on November 2-6, 1981, it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR Part 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Section 6.3 of the Gibbs and Hill (G&H) Comanche Peak Steam Electric Station (CPSES) project procedure DC-5 (Preparation of Procurement Specification Procedure) states in part that, ".
revisions to the specification shall be conspicuously identified line bars denoting the revision number shall be placed in the right hand margin to set off the specific text change."
Sections 5 and 6 of G&H CPSES project procedure DC-5 require that the edited and finished typed document (the original issue and all revisions thereof) be approved and signed off by the Chief Engineer and then routed for final review, approval, and sign off by the affected disciplines, the design reviewer and finally, by the CPSES Project Manager.
Contrary to the above:

Line bars denoting the revision number were not placed in the right hand margin of Revision 3 of specification 2323-MS-46A (Nuclear Safety Class Pipe Hangers and Supports) to set off the specific text changes made in paragraphs 3.6.2.1.1.g.1, 3.6.2.1.1.g.2, 3.6.2.1.1.g.3, 3.6.2.1.1.h.3.(a),
3.6.2.1.1.h.3.(b), and seven others.

The edited and finished typed document (Revision 3 of 2323-MS-46A) sas not reviewed by the design reviewer.
