Dear Mr. Collins:
As a result of the investigation and analysis of the January 23, 1978 i
incident (reference Reportable Occurrence Report No.
50-267/78-03/01/T-0),
Public Service Company initiated two programs to improve the reliability of the circulator auxiliary system (System 21) and to reduce the interaction of the two loops in the system.
These two 5
programs are:
1.
An annual calibration and functional test of selected instruments in System 21.
2.
A modification of the System 21 buffer helium return piping that separates the two loops into essentially two independent systems.
l The calibration and functional testing program has been conducted annually since November of 1978.
During this period the performance of the functior,al testing has net led to the discovery of any instrument malfunctions or incorrect instrument calibrations.
Instruments that have developed a malfunction have been discovered eitner at the time of the ralfunctior, due to its affect on system operation, or at the time of instrument calibration.
The modification of System 21 buffer helium return piping is currently under construction and will be operational the next time the plant is brought up to power. This modification will greatly reduce the consequences of the type of malfunction that occurred on January 23, 1978. An upset or malfunction in one loop of the system

j will have no common flow path to transmit the disturbance to the other loop.
Based on the above, PSC proposes that the four step program covering l
System 21 calibrations and functional testing outlined in the letter ht g

I. (G-78102) dated November 1,1978, Karl V. Seyf rit to C. K.Millen., be modified to require maintaining the following:
1.
Calibration of Safety Related instrument seismic types 1 and 2 (formerly identified as Class A instruments) on a periodic basis as defined by plant procedures.
2.
Calibration of all instruments required by the Technical Specifications (formerly identified as Class C instruments) on a
periodic basis as defined in the Technical Specifications.
3.
Calibration of all other instruments in System 21 (formerly identified as Class B and D instruments) on an as required basis.
4.
No functional testing of instrumentation except those that are iricluded as part of surveillance procedures.
