Appendix NOTICE OF VIOLATION Northern 5;ates Power Company License No. DPR-42 License No. DPR-60 As a result of the inspection conducted on December 14-18, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
10 CFR 55, Appendix A states in part, "The requalification program shall include on-the-job training so that:...Each licensed operator and senior operator is cognizant of facility design changes, procedure changes, and facility license changes."
A letter from Northern States Power Company (NSP) to the NRC, dated August 1, 1980, which contains the NSP response to the March 28, 1980, letter f rom II. R. Denton to all licensees states in part, "This program shall ensure each licensed individual is cognizant of plant changes, modifications, and reportable occurrences..."
Contrary to the above, 12 licensed operators and senior operators failed to conduct assigned reviews of plant changes, procedure changes, or ap-plicable reportable occurrances. This occurred for required reviews covering the period from June to September 1981.
Two individuals had five or more required reviews during this period for which no review has been documented.
This is a Severity Level VI violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affi rmation.
Consideration may be given to extending your response time for good cause shown.
