PVNGS UNIT 2 l

I.

Description of Deficiency The defect exists only in the #1 cylinder head of one i

diesel generator (2-M-DCA-Il01) furnished by Cooper I

Energy Services for PVNGS Unit 2.

The 13/16" drilled hole where the fuel nozzle tip emerges through the head was drilled oversize during the process of manu-facturing. This hole was repaired by Dalic copper l

plating the hole to size.

Susquehanna Steam Electric l

Station tests of a similarly repaired unit discloaed that this plating can fail due to the latte difference between the coefficient of expansion cf the copper i

plating and the parent metal, and the large temperature fluctuations that occur in this part of the head.

II.

Analysis of Safety Implication l

The condition is evaluated as a significant safety hazard and, therefore, is Reportable since a failure f

in this area of the head could cause the fuel injection nozzle to be blown out of the cylinder head and a top-side engine fire could result.

Corrective Action Cooper Energy Services has notified Bechtel that the j

defective head will be replaced prior to start-up.

The modificat taa will be accomplished /ta NCR MG-991.
