  • The Seabrook Station PSAR for Units 1 and 2 states, i n pa rt, in paragraph 17.1. 5 t ha t :

"Each organi zat ion is required to perf orm t hei r respect ive quallr y related activit ies covered by this program in accordance with docunented instruction, p roc ed u re s, o r d ra w i ng s. "

UEAC d rawi ng ?!805169S, as revised by Engineering Change Authorizations (ECA) 08/1197A and 25/0608A, provides details in sketch SY,-5/14/81-AF for the support of the Conbust i ble Gas Cont rol (CGC) inst rument tubing at 36 support points inside the Unit I containment dome a rea.

This i s a Se ve ri t y Le ve l IV Violation (Supplement II).

Pesponse:

Correc t ive Ac t ion Taken and Result s Achieved Ui:6C Engineering Change Approval (ECA) 08/1197A, dated May 14, 1981, necessitated the p roc u reme n t of clamps wi th special SA-325 bolts for use in the containment dome.

Johnson Cont rols (JCI), the instrumentation and control cont rac t or, prepa red a requisition for the material which was forwa rded t o UE6C (const ruc t ion manager) Home Of fice for ac t lon.

UE6C, in turn, ve rba lly ordered the clamps wi th SA-325 bolts and requested that the order be on site by July 1, 1981.

The ha rd wa re (st anda rd bolt s vs. SA-325) was received by the site rec e i vi ng inspection group prior to their rec e i p t of a copy of the change order containing the information that the vendor was to supply standard clamps with special bolting.

Receiving i nspec t ion was pe rf ormed usi ng the product specification as the acceptance standa rd vs. the change o rder specia l requ i reme n t s.

When JCI recei ved the i nc o r rec t ha rdwa re from UE6C, they presumed that it had been properly ordered and receipt inspected and subsequently installed the hangers.

JCI issued their NCR No. 002 (10/16/81) when they became aware of the p ro b l e m.

Correct bolting has been reordered and will be installed when the hangers become accessible.

The dome, f abricated at ground level, is now in place atop the containment.

Yankee will track this condition by use of its' future ve ri f icat ion items system to assure that this condition i s correc ted.

UE6C has conducted training sessions for all parties concerned where it was emphasized that their act ivi t ies must be conducted using proper documentation (i.e., specification, pu rcha se o rd e r, c ha nge o rd e r s, bills of material) and not verbal Instruction.

JCI has Instructed their personnel of the importance of checking material requi rements when prepa ri ng work inst ruct ions prior to installation.

Co r re c t i ve action will be completed when the correct bolting is received and the af f ec t ed ha ngers become accessible f or change-out.
