Artificial Enhancement of Radiographic Film (D/N #41)

Potential Problem On October 12, 1981, italpractice in the form of mechanically enhanced pene-trameter images was identified on station piping spool radiographs sub-aitted to the Ebasco Quality Assurance Engir.eering Department f1DE Group by Associated Piping and Engineering Corporation (AP&E), Compton, Califor-nia.

On October 16, 1981, the deficiency was documented on an fiCR which was sent to AP&E via telecopy, followed with the original by mail.

Proposed Resolution / Corrective Action The following actions are to be taken to establish the scope of the arti-ficial enhancement and to identify the affected spools for rework:

(1) AP&E to establish the cause and extent of the identified malpractice.

(2) AP&E to review all radiographic film for the period identified by

,the above action.

(3) AP&E to re-radiograph all welds with enhanced film in their possession and resubmit to Ebasco for review.

(4) Ebasco Site f4DE personnel to review all AP&E, Compton radiographs currently on site to determine whether enhancement had occurred on earlier radiographs and if so, to identify affected welds.

l (5) Any affected welds on spools on the Project Site are to be documented on an fiCR.

These welds are to be re-radiographed by an flDE Contractor, and submitted to Ebasco for review.

Status of Proposed Resolution / Corrective Action j

(1) The AP&E Level II interpreter has admitted to performing the enhance-1 ment during a period of four to six weeks prior to the discovery by I

Ebasco.

The questionable film primarily concerned thin wall stain-less materials.

The interpreter has been discharged from his duties.

Subsequent investigations ha.ve revealed that the mechanical enhance-ment actually took place from liay 15, 1981 to October 12, 1981 and only one individual had taken part in the matter.

1 (2) AP&E reviewed all radiographic film in their possession regardless of performance date and identified those spools requiring re-radio-graphy.

Ebasco also reviewed these films and concurred with AP&E.

(3) AP&E has re-radiographed identified spools currently in their custody and these radiographs have been reviewed and accepted by Ebasco.

(4) Ebasco Site fide Personnel have reviewed all AP&E - Ccmpton film on j

Site and those spools with enhanced radiographs have been identi-i fied on an f1CR.

This review also determined that the problem dated l

back to fiay 15, 1981.

(5) A Site f4DE Contractor has been directed to re-radiograph all but two of the spools. AP&E has substituted enhanced film of these spools with acceptable file film.

l Final Report Delay i

Since the affected pipe spools are in the possession of-both the vendor and site contractor, considerable interface effort is required to re-solve the problem.

Consequently, additional time is required to com-plete the proposed resolutions. A final report will be provided by i

March 31, 1982.
