Dear Mr. Keppler:
This is to confirm my telephone report to Mr. E.R. Swanson, NRC Region III Inspector, at 1815 hours0.021 days <br />0.504 hours <br />0.003 weeks <br />6.906075e-4 months <br /> on January 7, 1982, at which time I re-ported the following:
1.
That during an internal review of the documentation associated with our Design Change RFC DC-12-2444 (Reactor Vessel Water Level Moni-toring System), it was discovered that a breakdown had occurred in our internal controls allowing the complete installation of this Design Change which had only been authorized for partial installa-tion.
2.
I also reported that during further review by AEPSC Fire Protection personnel, it was discovered that one of our single fire doors leading to our Sample Room did not have the required documentation to support it's UL classification.
As required by Technical Specification Section 6.9.1.81, a followup report will be submitted by January 20, 1982.
