1.0 IURPOSE 1.1 This procedure establishes the manner of authorizing work on permanent plant equipment and facilities, and portable support equipment at the Grand Gulf Nuclear Station (GCNS).

1.2 Revision four (4) of this procedure eliminates the use of Trou6Le Tickets at GCNS. Upon issuance of this precedure, Administrative Procedure 01-S-07-2, Trouble Tickets, is cancelled.

2.0 RESPONSI3ILITIES 2.1 The Maintenance Superintendent is responsible for the following:

2.1.1 Insuring that all saintenance work performed en permanent plant equipment and facilities have been properly authori ed and is carried out in a timely manner.

2.1.2 The operation of the Trouble Desk.

2.2 Each Section Supervisor is responsible for establishing the nethod within his section for authorizing work en any portable suppor: equipment under his centrol.

2.3 The Operations Shif t Superintendent is responsible foe:

2.3.1 Releasing all permanent plant equipment and f acilities (except that equipment located in the Administration Building, Warehouse, and Training Center) for maintenance work.

2.3.2 Release of all fire protection, security cemputer and engineering processor equipment, regardless of location, for maintenance work.

l 2.3.3 Ensuring that equipment returned to service is capable of performing its intended function.

2.4 The Technical Superintendent, through his delegated representatives, is responsible for:

2.4.1 Control and operation of the Maintenance Planning and Scheduling Systen and Maintenance Historical System.

2.4.2 Processing and tracking of Task Cards and MWO's.

2.4.3 Completion of failure inforsation for NpKD reporting and its review.

Examples are the Auxiliary Building, the Recirculation Pumps, the Condensate Storage Tanks, the Turbine, etc.

5.2 Portable Support Equipment - Equipment which is used by a group in support of its primary work function. The operation and function of such equipment is the responsibility of the group using the equipment.

Examples are laboratory equipment, vehicles, welding equipment, etc.

5.3 Maintenance Work order (MWO) - Form detailing the requirements for performing a specific task. The MWO is explained more fully in Reference 3.5.

5.4 Task Card - Card generated by the cemputerized Maintenance Planning ~ and Scheduling System (MPSS) for a specific activity. The Task Card is explained more fully in References 3.1 and 3.5.

5.5 Maintenance Planning and Scheduling Systen (MPSS) - A cceputeri:ed systes used to schedule repetitive or deforred activities. The systes generates the Task Cards as well as various reports.

5.6 Trouble Desk - An area for receiving calls for maintenance assistance for ite=s which are not plant safety related. All calls will be referred to the responsible Maintenance Discipline Superintendent, who will =ake the determination if the requested work can be handled by the Trcubir Desk.

Operation of the Trouble Desk is covered by Reference 3.5.

6.0 DETAILS 6.1 All work perior=ed on permanent plant equip =ent and facilities will normally be the responsibility of the Maintenance Section. This will be accomplished is either of the following =anners.

6.1.1 Perfor=ance of the work by Maintenance section personnel.

6.1.2 Perfor=ance of the work by personnel who are respensible to the Maintenarce Superintendent. These =ay be either MP&L personnel other than the GGNS plant staf f, or personnel who have contracted to perform specific activities.

6.1.3 All MPSS Task Cards with deficiency ec==ents, and MWO's shall be

screened for potential reportable deficiencies La accordance with Reference 3.7.

6.1.4 Any corrective actica initiated is to be indicated on the work doce=ent by the appropriate supervisor. Where applicable, the indication of correceive action should be a 'tield Report, MRO, etc., and its assigned n==ber indicated on the work docu=ent for each deficiency.

NOTE Routine repetitive jobs, such as changing indicator lights or fuses, taking up packing, oiling components, etc., does not require a MWO providing these activities do not affect the operability of the systes or the safety of the workmen and the work is performed by a qualified craf tsman or operator and approved by the immediate supervisor.

6.2 The mechaniss used for authori:ing any verk to be performed on per=anent plant equipment or facilities will be one of the following:

6.2.2 Approved Maintenance Work Order 6.2.3 Trouble Desk 6.3 Work on. portable support equipment may be perfor=ed by section personnel if it has been authorized by the appropriate Section Supervisor /

Superintendent respcasible for the equipment.

This work must be controlled by section procedures containing the same type controls listed in paragraph 6.5.2.b.

6.4 Repetitive work activities will be scheduled and initiated inr the computerized Maintenance Planning and Scheduling System (M?SS), and Task Cards will be issued for the work in accordance with Referetce 3.1.

6.4.1 Distribution of the Task Cards will be perfor=ed by the Technical Section.

6.4.2 All cempleted or rescheduled Task Cards will be returned to the Technical Section for processing.

6.5 Work activities of a nonrepetitive nature will nor= ally be initiated by a MWo.

6.5.1 All MWU's will be routed to the Technical Section, via che Shif t Supervis or/Superint endent.

6.5.2 The MWO will be initiated, processed, worked, and closed in accordance with directions set forth by the Maintenance and/or Technical Section La section procedures.

6.5.4 When repair / replacement parts and caterial are needed for work being performed on safety related syste=s, a determination of the safety classification of the part/caterial shall be decernined in accordance with Reference 3.8 (GGNS Quality Classifications). The responsible supervisor (supervisor in charge of the work) shall make this determination with assistance from Maintenance Engineering / Nuclear Plant Engineering, as necessary.

q 6.6 For maintenance assistance en ite=s which are not plant system related, the Trouble Desk is to be used.

6.6.1 Exa=ples of the types of work to be handled by the Trouble Desk are:

Proble=s in the Administracien 3uilding, 'Jarehouse, or Training Building, except Fire Protection, Engineering Processor and Security Cc=puter Systems

Re-lamping' in all areas, heating, air conditioning, etc.

Furnishing personnel to Startup, Operations, or Technical Section for assistance, where work / test instructions and data remais the responsibility of the requesting organization f.

6.6.2 Any verk or problem resolution, beyond those type given in step I

6.6.1, desired on plant systems shall be by submission of a MWO.

6.6,3 The responsible Maintenance Discipline Superintendent shall make the determination as to whether or not work can be handled through the Trouble Desk. He will notify the originator of the request, if a MWO is required.

6.7 The organization responsible for the equipment will release it for the perfor=ance of the work activity (See Reference 3.4). The supervisor responsible for the work activity is responsible for obtaining any required protective tags in accordance with Reference 3.2 or Radiation Work Permits in accordance with Reference 3.6, or any additional permits required to complete the activity.

6.8 Work of. an unanticipated nature which is required to be initiated immediately or en an off shift, will be initiated by the Shift Superintendent by issuing a MWo.

NOTE If notification is =ade by celecon, indicate in the appropriate space of the NWO that telecon notification was obtained i.e., "D. L. Eunt per telecon 5/7/81" or "R. Elms per telecen 5/7/81".

6.8.1 For safety related work, the Shif t Superintendent will perform the following:

Notify the designated Plant Quality representative of the work to be performed.

Notify the designated Maintenance Section representative of the required work.

c.

Complete the necessary portions of cards 10, 11, 12, 13, 14, and 15 of the X%0 to initiate the work. 'He =ay consult with the appropriate engineer / planner and/or the responsible Maintenance Supervisor / Superintendent as required.

d.

Give a reproduced copy of the MWO to the Maintenance Supervisor / Superintendent responsible for the work as his working copy.

e.

Forward the yellow copy to Plant Quality, f.

Forward golden rod copy to Health Physics.

g.

Retain the green copy.

Forward the original and all other copies to the Maintenance Planner for processing the next normal work day.

1 6.8.2 The Maintenance Planner, in conjunction with the appropriate engineer, will review the MWO, make any required entries, log in the MWO and nake distribution of the remaining copies.

6.9.1 Completion of work and special testing will be docu=ented on the NWO.

6.9.2 The cleanliness of the system is maintained La accordance with Reference 3.9.

6.9.3 Restcration of valve, breaker and switch line up will be docu=ented on the Equipment Clearance Sheet.

6.9.4 Documentation of required post restoration testing will be in the Control Room Operator's Log block.

6.10 All work documents on plant equipment, i. e., MWO ' s, cask cards, etc.,

under Startup jurisdiction (green tag phase) will be routed to the -

designated Startup Test Supervisor. The Startup. Test Supervisor will initial thd ATS Block prior to the Shif cWeiincendent's 1.hi-iailing

approving the ccamencing of work and he will also initiate the RTO 31ock upon completion of_ the work-activity. prior to the initia1. ling by the Shift Sup'erintendent. Any required corrective action vill be initiated by the St'artup Test Supervisor and documented in accordance with Startup procedures.

6.11 Upon receipt of approval of written Stop Work Orders, the person receiving the Stop Work Order shall co= ply with the ceder and promptly notify his i==ediate supervisor. This notification shall tentinue up the line of supervision to the Plant Manager. The respcasible section supervisor shall ensure that the stopped activity is restarted only when authorized by the Plant Manager er the responsible (Section)
