Dear Mr. O'Reilly:
Nbh This is to confirm the telephone call to Mr. Paul Kellogg (NRC) at approximately 0900 on December 26, 1981 by Mr. W. G. Hairston, III (APCo) in accordance with Unit 2 Tech. Spec. 6.9.1.12.f.
At 0020 on 12/26/81, it-was determined that the Unit 2 containment spray additive tank isolation valve (V024) was locked in the closed position rendering the containment spray additive tank inoperable. The valve was immediately opened and locked open. The additive tank was returned to service at approximately 0040 on 12/26/81.
An investigation is underway in an attempt to determine when and why the subject valve was closed. Further details will be provided in the folloup report.
