This letter provides notification of Reportuble Occurrence 81-41 p T ant' to the requirements of Millstone Unit 2 Appendix A Technical Specifications, Section 6.9.1.8.C.
This report concerns the discovery of abnormal degradation of the reactor coolant system pressure boundary, specifically the steam generator tubes.
Ongoing eddy current examinations have identified a number of tubes that contain defects which require that both steam generators be placed in the C-3 inspection category.
Sample sizes in both steam generators are being increased consistent with Technical Specification Table 4.4.6.
The defects range from 40% to nearly through wall penetration, and they appear to be of a pitting nature vice wastage or tube thinning.
Though data reduction is not complete, the frequency of occurrence is primarily on the cold leg side; and thus far, the defects are located between the tube sheet and the first egg crate support.
Further information will be provided in the two week report and as required by Technical Specification Section 4.4.5.1.5.C.
