T APPENDIX A NOTICE OF VIOLATION Fietropolitan Edison Company Docket No. 50-320 Three Mile Island Unit 2 License No. DPR-73 As a result of the inspection conducted September 27, 1981 through November 14, 1931, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1930) the following violation was identi fied.
The Order for !!odification of License, dated July 20,1979, as amended by the Order dated February 11, 1980, states in part:
Pending further amendment of the Facility Operating License, the licensee shall maintain the facility in accordance with the requirements set forth in Attachment 1..." (proposed Technical Specification, Appendix A to License No. OPR-73). The proposed Technical Specifcation 6.8.1.a states in part: "Uritten procedures shall be...iaplemented covering...the applicable procedures recommended in Appendix ' A' of Regulatory Guide 1.33, Revision 2, February 1978...." Appendix ' A' of Regulatory Guide 1.33 lists procedures for control of radioactivity and procedures for limiting personnel exposure.
Station Operating Procedure 2104-4.55, Re71sion 4, dated September 24,1980 (now Revision 5, dated September 29, 1981),
" Reactor Building Entry and Pre-Decon," paragraphs 4.3, 4.4, and 4.5 (page 5.0) requires in part that all entry tasks and RWP requirements be reviewed by Radiological Controls Staff, that an RWP be obtained in accordance with Station Operating Procedure 1613,
" Radiation '.iork Permits" (now Radiological Controls Procedure 4122),
and that the entry team, support Health Physicists, and support personnel have reviewed the Pd!P.
Contrary to the above, personnel involved in an unscheduled reactor building entry on September 3,1931, failed to adequately review RWP No.1866 in that no Radiological Control Technician z
accompanied the individual who made the entry, as required by item 13 of the RWP.
Contrary to the above, personnel involved in the unscheduled reactor building entry on September 3,1931, failed to adequately review RW? No.1866, in that an item of contaminated equipment was removed from the reactor building, a task not authorized by the work description, item 5, of the RWP.
This is a Severity Level IV violation (Supplement I).
