CPPR-134 As a result of the inspection conducted on October 1 and 9,1%1 and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1930),
the following violation was identified:
Criterion V of Appendix B of 10 CFR 50 states, in part, that:
" Activities affecting quality shall be prescribed by documentad instructions, procederes, or drawings....(which) shall include appropricte o"sntitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Section 17.1.5 of the PSAR for the Washington tiuclear Project (Jnit #1 describes that measures would be established tn comply with the above requirement.
Contrary to the above requirements, on October 9,1981, welding and leld inspection procedure JAJ-WI-010.1 did not include appropriate acceptance criteria for welding of skewed joints of piping support structural steel. This situation has existed since at least October 9,1979, when the contractor had contacted the AS!!E code comittee to inquire about this matter.
In the absence of specific AS!4E requirements, the contractor failad to define inspection criteria and techniques for assessing adequacy of,feld sizes of skew joints.
On October 9,1931 a weld of less than 1/8-inch thfoat size had been accomplished and accepted by the JA Jones Construction Company, whereas a 1/4-ine.h size fillet weld was shown for the 150-degree skewed joint or.
support steel for the in-core monitoring system, weld number til of sheet 29A of drawing 9779-LL-419500.
This is a Severity Level V violation (Supplenent II).
