APPENDIX A Nuclear Valve Division Borg Warner Corporation Docket No. 99900289/81-02 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspecticn conducted on October 5-9, 1981, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:

Criterion V of Appendix B to 10 CFR Part 50 states, " Activities affecting quality shall be prescribed by documenteh instructions, procedures, o'r drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Nonconformances with these requirements are as follows:

(1) The Nuclear Valve Division (NVD) QA Manual Revision L, Section 9, para-graph 9.8 states in part, "... welding shall be performed in accordance-with the procedure (NPB or NPS) specified on the applicable manufacturing outline. "

NVD Nuclear Process Specification (NPS) 70046, Revision X Hardfacing of P Number 8 Materials Using GTAW Process, describes the welding and electrical characteristics to be used during the operation. The current range for 1/8-inch rod material used was indicated as between 70-100 amps-max.

l Contrary to the above requirements a welder was observed hardfacing a valve (50 12579-3) using a 1/8 inch welding rod and current of 140 amps.

This amperage should be used only for 3/16 weld material rods.

i (2) Nuclear Practice Bulletin (NPB) 11-8 Revision B, paragraph 4.3.3 states in part, "Upon receiving notification of a nonconformance deviation, the l

Contrary to the above, upon receipt of a Engineering Deviation Report, f

dated July 27, 1981 concerning a test conducted on a 8" gate valve, i

the QA Director failed to convene a special review committee as required by NPB-11-8, Revision B.
