As a result of the inspection conducted on "ovember 13-19, 1991, and in accordance with the Interi Enforcement Policy, 45 FR 56754 (October 7, 1930), tne following violations were identified:
A.
License Cond' tion 15 states that except as specifical'y provided otherwise by this license, the licensee shall possess and use licensed material in accordance with statements, representations, and procedures contained in part in letters dated Novenber 15, 1977 and March 20, 1079.

Iter ? of the letter dated March 20, 1978 states tnat the Radioisotope Committee will review the radiation protection program annually.
Cnntrary to the above requirement, the Radioisotope Comnittee did not review the radiation orotection progran durina 1979 and 1980.

Section 2 of the letter dated November 15, 1977 requires the Radiation Protection 1fficer to subnit to the Radioisotooes Connittee a re& ort on the Radiation Safety and luality Eurvey, which he conducts, on 'n annual basis.
Cnrtrar y to the 3bove requirement, at the time of the inspection a report on the Radiation Safety and Quality 5;rvc for the year looO had not been su5nitted to the Radioisotooes Committee by
'he Radiation Protection Officer.
The aoove itels constitute 3 Severity Level I,' Violation (Suoplem.ent VII).
R 1C CFD 35.14/e) states that any licensee who p ssesses sealed sources as calibration or reference sources pursuant to :aragraph /d) of this section shall cause each sealed source containing byproduct material, other than nydrogen-3, nith a half life greater thantthirty days in any for, other than gas to be tested for leakage and/or contanination at intervals not to exceed six months.
Ccntrary to the above requirement, three sealed sources; a 1.25 millicurie cobalt-60, Serial No. ICN-754, a 1.24 millicurie cestun 137, Serial No. IC5-755, and a 153 microcurie barium-133, Serial No. 319-144-07, were leak tested on Au,ust 21, 1900, and were not leak tested egain until April 9, 1931.
Also, at the ti"e of the inspect inn, these sourc s have not been leak tested since April 8, 1911.
Inis is a Severity Level V Violation (Supplement VII).

-10. CFR 20.401(b) requires that records of disposals made rursuant to 10 CFR 20.303 be maintained.
Centrary to the above requirement, at the time of the inspection, records of liquid waste disposal made to the sanitary sewerage system pursuant to 10 CFR 20.303 were not being maintained.
This is a Severity Level VI Violation (Supplement VII).
Pursuant to the provisions of 10 CFR 2.201, Veterans Administration Center, Reno, is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, intluding:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full conpliance will be achieved. Consideration may be given to extending your respense time for good cause shown.
