NOTICE OF VIOLATION' Baptist Memorial-Hospital' License No. 24-06806-01 i

As a result of the inspectica conducted on November.19,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),-the following violations were identified:

Item No. 14.D, states that all patient doses will be assayed just prior to administration in the Radx dose calibrator and the assay will be t

recorded on the patients prescription form.

Contrary to the above requirement, iodine-131 therapy doses were not assayed with the Radx dose calibrator prior to administration.

License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in the application dated August 24, 1977; and letters dated November 18, 1977, and. April 4, 1979. Application dated August 24, 4

1977, Item No. 14.B, states that procedures for examining incoming packages will be in accordance with the NRC Licensing Guide for Medical Programs dated February 1976. The Medical Licensing Guide states that for non-exempt packages the exposure rate at three feet, surface, and of the empty container must be taken; and a wipe test of the final source container must be performed.

Contrary to the above requirement, incoming packages containing iodine-131 therapy solution of more than one hundred millicuries have not been surveyed since 1978.

This is a Severity Level VI violation (Supplement VII).

Appendix Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:

.(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the authority-of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.

Consideration may-be given to extending your response time for good cause shown.
