'x Item 10 of this, application states that the procedures described in Appendix D Section 2 of the NRC Medical Licensing Guide, NUREG-0338, Revision 1, will be used for calibration of the dose calibrator.

Appendix D requires that quartecly linearity checks of the dose calibrator be performed and recorded.
Contrary to this requirement, no linearity checks have been performed on your new dose calibrator, put into service in early 1981.
This is a Severity Level V violation (Supplement VII).

License Condition No. 19 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained ~1n the application dated August 7, 1978.
Item 10lof this application states that survey instruments will be calibrated at least annually and following repair.
Contrary to this requirement, your Victoreen Model 444, Serial No.
f389, was last calibrated on May 23, 1979, a period of excess of one

10 CFR 35.14(e) requires that sealed calibration or reference sources possessed pursuant to 10 CFR 35.14(d) be tested for leakage and/or contamination at intervals not to exceed six months.
Contrary to this requirement, as of the day of the inspection, you have not leak tested your nominal 214 microcurie cesium-137 sealed reference source since April 1980, a interval of more than six months.
This is a Severity Level V violation (Supplement VII).
