This refers to the routine safety inspection conducted by Mr. W. J. Slawinski of this office on November 19, 1981, of activities ~ authorized by NRC Byproduct Material License No. 21-12930-01 and to the discussion of our findings with you and Ramanik Patel at the conclusion of the inspection and to the tele-i phone conversation with Marguerite Nowitzke on November 24, 1981.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with

personnel.
During this inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in the enclosed Appendix.
A written response, submitted under oath or affirmation, is required.

.i Also enclosed is information requested by Mr. Patel regarding emergency handling of radiation accident cases including an example hospital plan for establishing such a program.
