r) iscussion was office and yourself on October 7, 1981. The subject o the fact that NRC records show that the Special Nuclear Material License identified above has expired and no renewal application has been filed in accordance with Section 70.33 of Title 10, Code of Federal Regulations, Part 70.
From our discussions with you, we learned that you do not have any licensed i
material in your possession, nor have you ever received any licensed material under this license.
Therefore, you must prepare a statement confirming that from the dete the license was issued until the date it expired, no licensed material was acquired, received, possessed or used and that no patients were implanted l
with nuclear pacemakers at your hospital.
A copy of the statement shall be sent, within thirty days of the date of this letter, to Region I, U.S. Nuclear Regulatory Commission, Attention:
Chief, Materials Radiological Protection Section, 631 Park Avenue, King of Prussia, Pennsylvania 19406.
We request that you give prompt attention to this matter.
