Dear Mr. Sreniawski:
In reply to your letter of July 14, 1981, concerning items of noncompliance, the following actions have been taken:
Item 1.
Wipe tests for contamination of the sealed calibration sources have been perfomred and will be performed at six month intervals.
I do question the need for testing of these low level sources (200 FCi) since the radioisotope is sealed in inert epoxy which is not subject to leakage. Can you tell me how many incidences of leakage have been recorded and under what circumstances? If the source is broken, then certainly, wipe testing is necessary before source is used.
Items 2 and 3.
Personnel have been instructed to check dose calibrator with Cs-137 and Co-57 sources, and to perform wipe tests and survey weekly as required.
Item 4.
Emergency rules have been posted in the Nuclear Medicine Lab where radioactive materials are used.
Item 5.
Monthly spot checks were not performed when ionization chamber and electrometer were at Victoreen Calibration Lab for calibration.
Item 6.
Birthdates will be added to radiation exposure records.
Item 7.
Enclosed is a copy of service inspection report.
Concerning License Number SNM-1393:
Items 8 and 9.
Patient follow-up procedures have been established and will be conducted on remaining nuclear pacemaker patients; loss of contact with patient will be reported.
Item 10. Nuclear powered pacemakers utilizing nuclear materi:1s are no longer implanted at this hospital.
Personnel d6 not handle nuclear material in the Cardio-vascular Study Department. Therefore, no documents or notices are posted.
