4 APPENDIX A NOTICE OF VIOLATION Baltimore Gas and Electriccompany Docket Nos. 50-317 saltimore, Maryland 21203 50-318 As a result of the inspection conducted on January 5-9, 1981, and in accordanco with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violation was identified:

Technical Specification (T.S.) 6.11. " Radiation Protection Program", states:

" Procedures for personnel radiation protection shall be prepared consistent.

with tne requirements of 10 CFR Part 20 and shall be approved, maintained and adhered to for all operations involving persennel radiation exposure."

Rad-Chem Procedures RCP 3-310, revised February 14, 1980, and approved j

and maintained pursuant to T.S. 6.11, states in Section 4.4:

" Exposed QA TLD chips are exposed to sources which will produce absorbed doses I

which are reproducible.

NOTE-Normally some of these chips are exposed usin;, Cesium-137 Calibration Facility and others are only exposed to the Depleted Uranium Sources."

Contrary to the above, as of January 8,1981, no TLD chips were being exposed to the Depleted Uranium Sources.

Rad-Chem Procedure RCP 3-310, revised February 14, 1980, states in sections 6.3 and 6.4: '6.3 - Whenever an abnormal dosimetry evaluation is performed, an Expostre Evaluation Report, RCP-3-310-4, shall be prepared by the RSCF and submitted to the RSCE for approval.

Copies of RCP-3-310-2, RCP-3-310-3 and RCP-3-302-1 shall be attached when appropriate. 6.4 - RCP-3-310-4 shall be filed in the individual's radiation exposure records."

Contrary to the above, as of January 8,1981, the form RCP-3-310-4 had not been placed in the radiation exposure records for the 23 individuals listed on the September 1979 rosimeter Discrepancy Evaluation.

This is a Severity Level V Violation.

Pursuant to the provisions of 10 CFR 2.201, Baltimore Gas and Electric Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged violation; (2) the corrective steps which have been taken and the results achieved; (3) corrective steps which will be taken to avoid further violations; and (4) the date when full compliance will be achieved.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
