Appendix A NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-10 Docket No. 50-237 Docket No. 50-249 Based on the inspection conducted on October 18, 20, 21, and 26-28, 1977, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
Items 1 and 2 are infractions, and item 3 is a deficiency.
The deficiency requires no further response.
1.
Contrary to 10 CFR 50, Appendix B, Criterion V, and the licensee's Quality Assurarra Program, the licensee, on two separate occasions, allowed safety related modifica-tion work on Unit 2 to be completed prior to proper management reviews, as identified in QP 3-51 and DAP 15-1.
2.
Contrary to 10 CFR 50, Appendix B, Criterion V, and the licenmee's Quality Assurance Program, the licensee failed to properly control Unit 2 modification package No. 2-76-55 in that the package is lost. Quality procedure No. 17-51 identifies modification packages as lifetime records.
3.
Contrary to Section 6.5 of the licensee's Units 2 and 3 Technical Specifications, records of normal plant operation, in the form of roll charts, were not available for review by the inspector. The lack of records included portions of the IRM and APRM charts, vessel level and feedwater con-trol charts and air ejector offgas charts for the year 1977.
Section 6.5 of the Technical Specifications require these records be available for review and retained for a period of 5 years.
