NRC EXAMINATION REPORT N0. 50-413/93-300
This refers to the Nuclear Regulatory Commission (NRC) administered
examinations led by James H. Moorman, III during the period May 11-14, 1993.
The examinations were administered to employees of your company who currently
,
hold licenses to operate the Catawba Nuclear Plant. At the conclusion of the
examination, the examinations and preliminary findings were discussed with
those members of your staff identified in the NRC Examination Report,
Enclosure 1.
A Simulator Facility Report is included in this report as. Enclosure 2.
A copy
of the initial retake and requalification written examination questions and
'
answer key, as noted in Enclosure 3 and 4, was provided to members of your
training staff at the conclusion of the examination.
This is the third time in as many years that the NRC has found your licensed
operator requalification program to be borderline satisfactory. Any decrease
t
in the effectiveness of your licensed operator requalification training
program could reduce the margin of safety provided by properly trained
operators and result in the program being judged unsatisfactory by the NRC.
While improvement was noted in areas that were identified as weak during last
year's examination, operators taking this examination exhibited weaknesses in
another area.
As demonstrated to our examiners during the walkthrough portion
of the exams, there was a pervasive attitude among the operators that
performance of non-emergency evolutions need not necessarily be accomplished
in accordance with procedural guidance.
This lack of attention to detail in
the implementation of procedures is consistent with past performance of
personnel at your facility as indicated in your most recent SALP evaluation.
Future performance in this area will be closely monitored by the NRC.
You are requested to respond to this letter detailing your corrective actions
to the concerns expressed above.
Please provide this response within 30 days
of receipt of this letter.
Our examiners noted an improvement in the_ quality of your Emergency Operating
Procedures (EOPs) and in the operator's ability to use these procedures. They

also noted that the communication between operators during the implementation
of the E0Ps, while significantly better than last year, still has room for
improvement.
