SUBJECT: NRC INSPECTION REPORT NOS. 50-327/93-13 AND 50-328/93-13

This refers to the inspection conducted by W. Holland of this office on

!

April 4,1993 through May 1,1993. The inspection included a review of

~

i

activities authorized for your Sequoyah facility. At the conclusion of the

inspection, the findings were discussed with those members of your staff

L

identified in the enclosed inspection report.

,

Areas examined during the inspection are identified in the report.

Within

these areas, the inspection consisted of selective examinations of procedures

-and representative records, interviews with personnel, and observation of

i

activities in progress.

'

The enclosed Inspection Report identifies activities that violated NRC

,

requirements that will not be subject to enforcement action because the~

-

licensee's efforts in identifying and/or correcting the violation meet the

criteria specified in Section VII.B of the Enforcement Policy.

,

We are concerned with your lack of sensitivity-to an evolution important to

safety such as draining down of the Reactor Coolant System. This concern is

i

.

described in paragrpah 3.a of the report. This lack of sensitivity was

'

exhibited by the operators not recognizing the cold calibrated pressurizer

level instrument was indicating improperly, and not ensuring the reliability

,'

of the cold calibrated pressurizer level channel prior to use.

In addition, a

decision w n made not to have the Reactor Vessel Level Indication System

available as a backup, which hampered operations' ability to accurately

determine reactor water level.

You have implemented corrective actions

i

appropriate to addressing these concerns. We concur with these initi.1

actions and will continue to monitor your performance in this area during

{

future inspections.

In accordance with Section 2.790(a) of'the NRC's " Rules of Practice," a copy

'

of this. letter and its enclosure will be placed in the NRC's Public Document

Room.
