SUBJECT: INSPECTION NO. 50-309/93-07
This transmits the March 23 through April 27,1993, inspection findings of Messrs.
C. Marschall and W. Olsen at the Maine Yankee Atomic Power Plant, Wiscasset, Maine.
Those findings were based on observations of activities, interviews, and document reviews,
and have been discussed with Mr. Blackmore of your staff.
During the inspection, we noted safe plant operation and maintenance. Especially noteworthy
l
was the plant staff response to the increasing steam generator primary to secondary leak rate.
The root cause evaluation for the AK-25 circuit breaker problem was thorough and indicated
excellent engineering department investigation. However, inadequate post-maintenance
system turn-over resulted in isolation of tha steam generator blowdown system radiation
monitor. This violation of the Offsite Dose Calculation Manual requirements will not be
subject to enforcement action because you identified and promptly corrected it and met the
other criteria of the enforcement policy in 10 CFR Fart 2, Appendix C,Section VII.B.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice", a copy of this letter and
its enclosures will be placed in the NRC Public Document Room.
