SUBJECT: NRC INSPECTION REPORT NO. 50-382/93-10 (NOTICE OF VIOLATION)

This refers to the special inspection. conducted by'Mr. S. J._ Campbell of this
i
office, during the period March 24 through Apr_il 1,1993. The inspection:
included a review of activities authorized for your Waterford Steam Electric
i
l
Station, Unit 3, facility. At the conclusion of the1 inspection,-the findings-
l
were discussed with those members of your staff identified in the enclosed
i
'
report.
j
Areas examined d'uring the inspection are identified in the report. . Within
l
-
these areas, the inspection consisted of selective _ examinations of procedures
,
and representative records, interviews with personnel, and observations of.
,
,
activities in progress.
j

i
Based on the results of this inspection, certain licensed activities appeared
!
to be in violation of NRC requirements, as specified in the enclosed Notice of
-
Violation (Notice). The violation is of concern because it appears that
.
l
!
hourly fire watch patrols were inadequate to properly identify the presence of
fire hazards.
.
It appears that further evidence of the inadequacy.of.your'. fire watch-patrols
I
was identified by our resident inspectors in a violation discussed in NRC
i
Inspection Report 50-382/92-23.
In that instance, your fire watch patrols
passed transient combustibles in the vestibule outside of Switchgear Cage AB
i
on numerous occasions and failed to identify the hazard. While.we realize
,
that this area was not specifically. identified as an area included on the
i
impairment logs, the fact that these' patrols did not identify this condition.
,
as a fire hazard is also of' concern. As discussed between Mr. K. M. Kennedy
of my staff and Mr. Laughlin of your staff on May 4,1993, we have scheduled a
management meeting for May 14, 1993, to discuss the effectiveness of your fire
watch patrols and the control of transient combustibles.
You are required to respond to this letter and should follow the instructions
i
specified in the enclosed Notice when preparing your response.
In your
i
response, you should document the specific actions taken and any additional
actions you plan to' prevent recurrence. After reviewing your response to this
Notice, including your proposed corrective actions and the results of -future

inspections, the NRC will determine whether further NRC enforcement is
!
necessary to ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR Section 2.790 of the NRC's " Rules of Practice," a
copy of this letter and its enclosures will be placed in the NRC Public
Document Room. The responses directed by this letter and the enclosed Notice
are not subject to the clearance procedures of the Office of Management and
Budget as required by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
Should you have any questions concerning this inspection, we will be pleased
to discuss them with you.
