SUBJECT:
MEETING WITH COMBUSTION ENGINEERING, INC. (CE) RE HEMATITE CONSOLIDATION - KARDEX CONFIGURATION On March 26, 1993, CE was authorized by License Amendment 23 to store special nuclear materials (SNM) in the Kardex unit. The number of storage shelves in the Kardex was limited to 360. During CE's "preoperational inspection," CE discovered that the actual number of storage shelves differed from that previously reported.
On April 14, 1993, CE representatives met with staff of the Office of Nuclear Material Safety and Safeguards and Region III to clarify this discrepancy.
In t
the meeting, CE provided detailed information on the safety implications, root causes, and actions taken to prevent recurrence (see enclosure).
By letter dated April 13, 1993, CE provided detailed information which also was discussed during the meeting. Although the discrepancy poses no immediate nuclear safety concerns, CE indicated that they will not introduce SNM into the Kardex until completing modifications to the "As designed" condition.
These modifications are to be completed by April 23, 1993.
CE has requested that NRC concur in the use of the Kardex as described in CE's April 13, 1993, submittal. The staff will respond to CE's request after the review is completed.

The Kardex Error The shelf spacing - 6 9/16" - was specified in the original 1989 hardware / installation procurement document.
The license amendment submitted in August,1992, based on this spacing, specified a maximum of 360 Kardex pans filled with pellets.
installation of the Kardex at Hematite was covered by a separate j
Purchase Order to the St. Louis representative.
This provided.for the initial installation of six modules.
The installer was given a copy of the original Purchase Order which specified 6 9/16'.' spacing.
The installer based the. spacing on parts-supplied -.not the Purchase Order specified spacing.
This resulted in 414 shelves at.5 s/8" rather than 330 at 6 9/16 A_B_B.
spacing.

. Inadequate communication between Combustion Engineering and the installer.
. tnadequate compensation for the delay between original procurement anc ultimate installation (including the impact of changing the installation vendor).
