SEQUOYAH NUCLEAR ~ PLANT UNIT 2 SPECIAL REPORT 93-03 FIRE DOOR A-41 Description of Event On: March 22, 1993, with Units 1 and 2 in Mode 5, auxiliary building Fire Door A-41 was breached to support the Unit 1 Cycle 6 refueling outage activities. Fire Door A-41 provides access to auxiliary building Room A-21, which previously contained the Unit 2 positive displacement charging pump. The room is currently empty and was being used.to rebuild seals for installation in the Unit 1 reactor coolant pumps. The door was breached to allow the routing of cables and high-efficiency particulate air filter hoses into the room. The door was breached in excess of the seven-day technical specification (TS) allowable time frame.
Cause of Condition The seal work was completed on Friday, March 26, 1993,'and all the cables and hoses were removed. However, the responsible foreman forgot to close the breach on Monday, March 29, 1993, because of involvement in other work activities.
Corrective Action The fire barrier was restored on April 1, 1993, at approximately 0900 Eastern standard time. The foreman involved was counseled on the event and informed of the expectations of management to fully comply with the TS action statements and requirements of the procedures governing fire barriers.
