NOTICE OF VIOLATION GPU Nuclear Corporation Docket No. 50-289 Three Mile Island Nuclear Generating Station - Unit 1 License No. DPR-50 During an NRC inspection conducted on February 23,1993 - March 27,1993, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below:
Technical Specification 6.8.1.a requires that written procedures shall be established, implemented, and maintained for the applicable procedures recommended in Appendix "A" of Regulatory Guide 1.33, Revision 2, February 1978. Appendix "A" of Regulatory Guide 1.33, revision 2, February 1978, item 3, requires that instructions for the operation of safety related systems such as the service water system (decay heat river water system) should be written to control startup, shutdown, and changing modes of operation.
Contrary to the above, the licensee failed to adequately establish and maintain Operations Surveillance Procedure S227, "DR-P-1A/B Periodic Surveillance," to periodically operate the decay heat river water system pumps to minimize the silt buildup under the punsp suction bowls. This deficiency resulted in both decay heat closed cooling water heat exchangers, DH-C-2A/B, being isolated and bypassed for approximately 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> and 55 minutes.
This is a Severity level IV violation (Supplement I)
Pursuant to the provisions of 10 CFR 2.201, GPU Nuclear Corporation is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, A'ITN: Document Control Desk, Washington, D.C. 20555; with a copy to the Regional Administrator, Region I, and a copy to the NRC Resident Inspector, within 30 days of the date of receipt of the letter transmitting this Notice of Violation. This reply should be clearly marked as a " Reply to a Notice of Violation," and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
