SUBJECT: NRC INSPECTION REPORT NOS. 50-289/93-09 AND 50-320/93-03
This refers to the inspection conducted by Mr. F. Young of this office from
February 23,1993 - March 27,1993. The inspection included a review of activities
authorized for the Three Mile Island Nuclear Station. At the conclusion of the inspection,
the findings were discussed with Mr. M. J. Ross and other members of your staff. Inspector
observations indicated safe and conservative operation.

Areas examined during the inspection are identified in the report. The inspection consisted of
document reviews, interviews, and observation of activities relevant to the health and safety
of the public. The purpose of the inspection was to determine whether activities authorized
by the licensee were conducted safety and in accordance with NRC requirements.
Based on the results of this inspection, certain of your activities appeared to be in violation of
.
NRC requirements, as specified in the enclosed Notice of Violation (Notice). Due to an
inadequate operations surveillance procedure, both trains of the decay heat river water system
were isolated / bypassed to the decay heat closed cooling water heat exchangers. You are
required to respond to this letter and should follow the instructions specified in the enclosed
Notice when preparing your response.
This report also discusses a licensee identified violation which is not being cited because our
review found that this issue was promptly raised to management attention, was of low safety
significance, and was followed up with prompt and effective corrective actions. The issue
involved the failure to follow the procedure associated with preventive maintenance on the
borated water storage tank level instrumentation heat trace. Our decision to not cite this item
is based on the NRC enforcement policy goal of encouraging licensees to aggressively and

thoroughly pursue self-identification and correction of problems.
