SUBJECT: NRC INSPECTION REPORT 50-482/93-03 (NOTICE OF VIOLATION)

This letter refers to the inspection conducted by Messrs. G. A. Pick and the

inspectors identified in the attached report for the period of February 14

through March 27, 1993. The inspection included a review of activities

)

authorized for your Wolf Creek Generating Station. At the conclusion of the

!

inspection, the findings were discussed with those members of your staff

identified in the enclosed report.

Areas examined during the inspection are identified in the report-. Within

these areas. the inspection consisted of selective examination of procedures

+

and representative records, interviews with personnel, and observations of

,

activities in progress.

Based on the results of this inspection. certain of your activities appeared

.l

to be in violation of NRC requirements as specified _in the enclosed Notice of

Violation (Notice). The violation includes two. examples of failure to

-i

properly control and coordinate clearance order activities. We are concerned

,

with these examples because of-the potential to injure personnel or to damage

,

equipment. We are aware that an investigation team was formed to review other

i

!

clearance order _ deficiencies.

In your response to the Notice of Violation,,

i

please address all enhancements you have made to address communication

deficiencies and program weaknesses-

l

,.

Wolf Creek Nuclear Operating Corporation personnel determined that personnel

failed to consider' the effects on containment integrity while implementing a

_[

procedure in a mode other than the ones for which the procedure was

!

specifically written. This violation of Technical. Specification 3.9.4 was not

i

,

l

cited because the criteria identified'in paragraph VII.B.2 of_10 CFR Part 2,

t

i

Appendix C (Enforcement Policy), were satisfied. Additionally, plant

!

personnel identified an instance of.failing to perform an 'as-found local leak

l

E

rate test because personnel disregarded the step in. work instructions that

j

required the test. This violation of Technical Specification 4.6.1.2. will not

i

be cited because the criteria identified in paragraph VII.B.2_ of -

d

10 CFR Part 2, Appendix C- (Enforcement Policy), were satisfied.

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.

In accordance with 10 CFR Section 2.790 of the NRC's " Rules of Practice," a

copy of this letter and its enclosures will be placed in the NRC Public

Document Room.

The responses directed by this letter and the enclosed Notice are not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
