Circle one, then enter corresponding document number; or enter "N/A" if there is no ERM, ECN, DDR, or FDDR associated with this DVSCN.

Place an X in this block if independent design verification is being deferred.

Place an X in this block if independent design verification has been completed, and list all DVSCN's that created the deferred status now being l
