SUBJECT: NOTICE OF VIOLATION
NRC INSPECTION REPORT NOS. 50-528/529/530/93-04
This refers to the inspection conducted by J. Sloan, H. Freeman, F. Ringwald,
F. Gee, and H. Wong of this office, on January 20 through February 22, 1993.
The inspection included a review of activities authorized for your Palo Verde
facility. At the conclusion of the inspection, the findings were discussed
with those members of your staff identified in the enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
Based on the results of this inspection, certain of your activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). The violation involves the failure to follow fire
protection test procedures.
We are also concerned regarding several configuration contrrl weaknesses which
were identified in this report. An instrument air isolation valve, which was
found improperly closed, contributed to causing a reactor trip in Unit 1;
portions of the Unit 2 auxiliary building became contaminated as the result of
the floor drain plugs not being installed as intended; and the design
documentation for circuit breakers in the auxiliary feedwater system differed
from installed field conditions which may have contributed to the failure of a
valve to open on February 4.
These examples, in conjunction with the
identified violation and several minor NRC identified deficiencies that should
have been identified by your staff, may indicate a need for greater management
emphasis on attention to detail.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence. After reviewing your response to this
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC requirements.

of this letter and the enclosures will be placed in the NRC Public Document

The responses directed by this letter and the enclosed Notice are not subject

by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
