SUBJECT: NOTICE OF VIOLATION

NRC INSPECTION REPORT N05. 50-528/529/530/93-09

l

This refers to an announced inspection conducted by Mr. M. J. Royack of this

office during the week of March 1 through 5,1993. The purpose of the

l

inspection was to determine whether activities authorized by the license were

l

conducted safely and in accordance with NRC requirements. At the conclusion

'

of the inspection the findings were discussed with Mr. G. Overbeck and other

members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress.

Based on the results of this inspection, one of your activities appeared to be

in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice).

The attached Notice identifies that on two occasions corrective actions, as

specified in material non-conformance reports (MNCRs), were documented as

being completed when they had not been completed as specified. Closing out of

corrective actions without assuring that the action has been completed is a

concern which requires management attention. We are concerned that this

situation may become a programmatic problem since a previous f RC Inspection

Report (50-528/529/530/92-43) also identified that corrective actions may not

have been fully completed or implemented as documented in closure documents.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific action taken and any additional

actions you plan to prevent recurrence. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.

procedures of the Office of Management and Budget as required by the Paperwork
