Elimination System (NPDES). permit. This-is accomplished by submitting copies of the reports required by the NPDES permit to the NRC.

l During January 1993 all sampled water sources were found to be within permit j
limits with the following exceptions. There was one instance in which two l
chemical limits were exceeded and one~ instance of a time limit being exceeded.
i i
Both violations occurred on January 23rd and stemmed from the same event i
involving the circulating water intake chlorination systems-for both Units 2 i
and 3.
These violations were reported ir. the-above referenced letters as required by the NPDES permit.
Per the above mentioned NRC reporting requirement, a copy of the two referenced reports are enclosed.

NPDES Monitoring Report San Onofre Nuclear Generating Station, Unit 3
Dear Mr. Coe:
The NPDES Discharge Monitoring Report (DMR) for San Onofre Unit 3 covering the month of January is submitted in accordance with the requirements of Order No.
85-13 (NPDES Permit No. CA0108181). A summany of the generating unit's status and significant analytical results is provided below.
The unit was in operation all month and all sampled water sources were found to be within permit limits with one exception. The measured total residual chlorine on the combined discharge on January 23 was 0.31 ppm. It was also determined that the reportable quantity value of 100 pounds of sodium hypochlorite was exceeded. An estimated amount of approximately 260 pounds of sodium hypochlorite was released.
All applicable regulatory agencies were notified. Because of inoperable equipment, Unit 3 was chlorinated with Unit 2 pumps.
During the cross chlorination, flow was mistakenly read from the Unit 2 flow meter.
Because some leakage existed past the Unit 2 block valves, the pump speed setting was increased to try and achieve the normal flow of 2 GPM, as indicated on the Unit 2 flow gauge. The flow to Unit 3 exceeded the normal flow rate and resulted in the violation.
1 Mr. Arthur L. Coe March 2,1993 Once the violation was discovered, the chlorine pump stroke was reduced and a subsequent sample taken 20 minutes later showed chlorine below permit limits.
Corrective actions were taken to restore the system to hormal configuration and schedule repairs. Administrative controls have also been put in place to prevent cross chlorination until the leaking block valves have been repaired. in addition, one of the Unit 3 chlorination pumps has been returned to service. It is believed that these actions will prevent reoccurrence of this problem in the future.

85-11 (NPDES Permit No. CA0108073). A summary of the generating unit's status i
and significant analytical results is provided below.
j The unit was in operation all month and all sampled water sources were found to be l
within permit limits. However, on January 23 the two hour chlorination daily time limit l
was exceeded due to leakage past Unit 2 block valves while chlorinating Unit 3 using j
a Unit 2 pump. Earlier in the day, Unit 2 was chlorinated for an hour and was witbic permit limits. Later that same day, the system was cross connected with Unit 2.
chlorination pumps supplying Unit 3. This was because the Unit 3 pumps were '
l inoperable. Unit 3 was chlorinated for one hou'r and thirteen minutes. The leakage j
past the block valves resulted in Unit 2 being chlorinated for a total of two hours and-i thirteen minutes for the day..Since that time,'one of the Unit 3 chlorination pumps has-been repaired and returned to service. The leaking valves have been identified.
Administrative controls have also been put in place to prohibit chlorinating a Unit using j
the other Units pumps until the leaking valves have been repaired.
