This refers to the inspection conducted by i . I. Barnes of this office on

Narch 5 and 17-19, 1993. The intpection int 3 ded a review of activities

authorized for your South Texas Project, Units 1 and 2, facility. At the

conclusion of the inspection, the findings were discussed with you and those

members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

I

activities in progress.

Based on the results of this inspection, certain licensed activities appeared

]

to be in violation of NRC requirements, as specified in the enclosed Notice of

1

- Violation (Notice). Violation A pertains to seven instances of boric acid

a

leaks that were not properly documented, contrary to the requirements of the

]

governing procedure. This violation is of concern because it is indicative of

i

insufficient attention to and oversight of conditions that have the potential

ig

to result in significant degradation of the reactor coolant system pressure

h

boundary.

Violation B pertains to two examples of inadequate corrective actions relative

to Unit 1 Steam Generator IB hot leg primary side manway cover boric acid

leakage and Unit I steam generator primary side manway stud elongation. This

$

violation is of concern because it represents additional examples in which

%

your corrective action program has been less than fully effective in resolving

k

significant hardware problems. Three other recent NRC inspection reports

T

(92-35, 93-05, and 93-08) documented several examples of safety-related

il

equipmelit problems that could be attributed, in part, to insufficient problem

identification, inadequate root cause analysis, or untimely and inadequate

j

corrective actions.
