Dear Mr. Boulette:
SUBJECT: PILGRIM INSPECTION 93-05
This refers to the safety inspection conducted by Messrs. J. Macdonald, D. Kern, and S. Hansell
of this office on February 11 to March 13, 1993 at the Pilgrim Nuclear Power Station,
Plymouth, Massachusetts. Areas relevant to the health and safety of the public examined during
this inspection are described in the enclosed report. Our findings were based upon observations
of performance and independent evaluations of safety systems and quality records.
The

preliminary results were discussed with you and other members of your staff at the conclusion
i
of the inspection.
Appropriate precautionary measures were taken in anticipation of the March 13-14 storm. Your
staff's immediate actions in response to the storm-related reactor trip were excellent, particularly
the controlled manner in which the Y-3 and Y-4 electrical buses were recovered after they had
unexpectedly tripped, and the overall command and control demonstrated by the Nuclear Watch
Engineer.
However, a post-trip review conducted after this inspection period found that
technical limits on temperature and pressure for the reactor vessel bottom head were exceeded
during the reactor cooldown. This event will be further evaluated by the NRC.
No reply to this report is necessary, and your cooperation with us is appreciated.
