Reply to a Notice of Violation Dear Mr. Slawinski In response to your letter on the special safety inspection conducted at Eli Lilly plant in Clinton, Indiana on X-Ray Co.
You found four violations associated with the radiographic field work, on November 6, 1992.
(1) Fs11ure to properly check alarm ratemeters prior to use and for proper function.

Reply corrected action I have tags with name, address, and phone numbers on them, attached to each exposure device.

November 6, 1992. Four violations associated with the radiographic field work on November 6, 1992 were identified and consist of failure to:
(1)prcperly check alarm ratemeters prior to use for radiographic operations, to ensure alarm function (Section 6); (2) post the high radiation area in which radiography was being performed (Section 3); (3) include all required information on shipping papers and properly mark transport packages (Section 9); and (4) attach a iabel to the exposure device bearing the licensee's name, address and telephone number (Section 8). Additionally, a concern associated with a radiographer's survey technique was identified ($ection 7).
